Giftcard Terms

1. Issuer

Gift Cards are issued by:
Invigorate Weight Loss & Wellness
Spartan Men’s Elite Health

For support or inquiries, contact your nearest clinic location or email:

The balance of the gift card will be applied to the wallet of the specified patient account.

2. Redemption

Gift Cards may be redeemed for eligible services and products offered by Invigorate Weight Loss & Wellness or Spartan Men’s Elite Health, including but not limited to:

  • Weight loss programs

  • Hormone therapy

  • IV hydration & vitamin injections

  • Peptide therapies

  • Lab services

  • Men’s hormone optimization services

  • Aesthetic services (where available)

Gift Cards cannot be used for:

  • Purchasing other gift cards

  • Membership dues or recurring charges unless specifically stated

  • Gift Cards may be redeemed in-clinic at participating Arkansas locations or online if/when digital redemption becomes available.

3. Expiration & Fees

  • Gift Cards do not expire.

  • No activation, maintenance, or inactivity fees apply.

  • Promotional or bonus gift cards (e.g., “Buy $100, Get $20 Free”) may contain separate expiration dates, which will be disclosed at the time of promotion.

4. Restrictions

Gift Cards may not be:

  • Redeemed for cash unless required by Arkansas state law

  • Returned or refunded after purchase

  • Transferred for resale or commercial purposes without permission

  • Used toward tips, gratuities, or cash-equivalent transactions

 

5. Balance Inquiries

To check a Gift Card balance, contact your local clinic or email:

6. Returns for Purchases Made With Gift Cards

If a service or product purchased with a Gift Card is refunded, the amount will be credited back to:

  • The original Gift Card, or

  • A new Gift Card issued by the Company

Refunds cannot be issued as cash.

7. Fraud & Unauthorized Use

We reserve the right to:

  • Refuse, cancel, or suspend any Gift Card

  • Close customer accounts

  • Decline transactions

If we believe the Gift Card was obtained fraudulently, used unlawfully, or violates these Terms.

8. Medical & Regulatory Disclaimer

Gift Cards may be redeemed for services that require:

  • Medical evaluation

  • Provider approval

  • Lab testing

  • Clinical eligibility

Receiving a clinical service depends on meeting all medical criteria determined by a licensed provider.
A Gift Card does not guarantee eligibility for any medical treatment.

9. Changes to Terms

The Company may modify these Terms without prior notice. Updated versions will be posted at:

Continued use or possession of a Gift Card constitutes acceptance of updated Terms.

10. Governing Law

These Terms are governed by the laws of the State of Arkansas, without regard to conflict-of-law provisions.
